Once we receive your referral,  whether its via telephone or via our website we will give you a call to arrange a face to face assessment with a care manager

Who will complete a detailed and robust care needs assessment and a risk assessment, ensuring all your needs are captured.

We believe in person centred practice. We want to work with you so it's important we understand your needs.

We will work with you, your family, next of kin and loved ones to ensure your assessment is a true reflection of your care needs.
